Please complete and sign the Waiver Release & Indemnity Deed (https://prodivecairns.com/pdf/waiver.pdf) and bring a copy of this and your dive medical, if required, with you when you check-in for your trip and Refresher course. Please note if you have booked a 2 Hour Refresher Course, it is a pool based program conducted seperately to your boat trip. It is scheduled on a different date from your tour/course, and the bus transfers are not provided Please make your own way to our training centre, which is located at 116 Spence Street by 9.00am. On arrival, you will be given a liability release and medical questionnaire form to complete. *IMPORTANT INFORMATION: If you answer 'yes' to any of the points below Pro Dive Cairns will require medical clearance from a doctor stating that you are 'fit to dive'. 1. Since completing your last dive medical have you suffered any illness, injury or surgery that may effect your ability to dive safely? Yes/No 2. Are you currently suffering any illness or injury? Yes/No 3. Are you currently taking any prescription medication other than the contraceptive pill? Yes/No 4. Is there any other condition, fact or circumstance that may affect your fitness or ability to dive safely? Yes/No Please note that you will be required to show proof of a PADI Open Water certification, or equivalent from another dive organization. If you are feeling a little rusty, we can organize a refresher course at our Training Centre in Cairns prior to the liveaboard trip (bookings at least one day prior are essential). If you would like to upgrade your certification please check out the courses we offer onboard under the courses tab on our website. The trip includes up to 11 fantastic dives (certified divers), 2 nights' accommodation and all meals on the boat, all gear hire, Cairns and Northern Beaches accommodation transfers (to as far north as Palm Cove). Pro Dive Cairns' certified dives are generally unguided. Guided dives may be available depending on availability of staff. Please note that towels are not included - we recommend that you bring along two towels each (one for the ocean and one for showering). Please reconfirm your booking on PH: 4031 5255 or call into our retail shop on the corner of Grafton and Shields streets at least 48 hours prior to your date of departure. This will also give you the opportunity to advise us of your accommodation details in Cairns so that we can arrange transfers for you. You can organise your gear at the retail shop anytime between 4:30pm – 6:30pm the day before the trip. If you are not able to confirm your pick up place prior to your departure date, you will need to make your own way to our retail shop for a 6:20am check in. 5 Day Learn To Dive :

Important Information:

Prior to commencing your dive course you are required to complete the self-assessment Student Divers Medical Declarations. A positive response to any questions indicates there is a pre-existing condition which may affect your safety while diving and you must consult a medical practitioner, preferably with experience in diving medicine, to obtain a dive medical certificate that states you are medically fit to dive.

To obtain a copy of the form please visit www.prodivecairnse.com/studentmedical.pdf. Medical must be complete within 90 days of starting your dive course.

We recommend that you not fly within 24 hours of your final dive. Your final scheduled dive of the 3-day trip will be complete by 12:00pm.

Cancellation and Refund Policy (5 Day Learn to Dive only):

You can cancel your Learn to Dive course for any reason up to the end of Day 1 of the course and receive a full refund of all money paid to Pro Dive Cairns. There is no refund for any reason after this time. If you have booked through a travel agent cancellation fees may apply. 

Note: This course is a nationally recognised training program and is delivered under partnership arrangements with PADI Asia Pacific (RTO ID 6729).

3 Day/2 Night Liveaboards  (inc Nitrox Advanced, PADI Advanced, PADI/Nitrox Advanced, 3 Day Nitrox package, Refresher Course, Introductory Dives):

Important Information:

Some medical conditions will prevent you from diving. You’ll be required to complete a statement about your current health and fitness to dive on board the vessel prior to any diving taking place.Please see the forms section of our website (http://www.prodivecairns.com.au/forms) or contact us for more information.

Please note that if you are diving, you will be required to show proof of a PADI Open Water certification, or equivalent from another dive organization. 

All passengers will also be required to complete and sign a legal Waiver Release & Indemnity Deed once on board the vessel.

We recommend you do not fly within 24 hours of your final dive. Your final scheduled dive of the 3-day trip will be completed by 12:00pm.

Cancellation and Refund Policy (3 Day/2 Night Liveaboards only):

You can cancel your dive trip for any reason up to 48 hours prior to departure and receive a full refund of all money paid to Pro Dive Cairns. There is no refund for any reason after this time. If you booked through a travel agent cancellation fees may apply.

4 Day eLearning.

The trip amount is fully refundable up until 24 hours before course commencement. The E-Learning portion of the course, AU$210 is non-refundable. 

Please make sure that you bring your printed PADI E-Learning Statement of Attainment, stating that you have completed all 5 modules of the theory components, this must be valid within 1 year.
